Request for a Certified Vital Record

REQUEST FOR A CERTIFIED VITAL RECORD 
OF AN OCCURRED
BIRTH / DEATH / MARRIAGE / CIVIL UNION / DOMESTIC PARTNERSHIP

To obtain a New Jersey State Certificate of a birth, death, marriage, civil union, or domestic partnership, which occurred in the Township of Evesham, you must complete an official Vital Statistics Request Form and submit same to the Evesham Township Registrar’s Office by either:

• Direct mail - with your payment, copy of proper identification, and a self-addressed stamped return envelope

 or

• Directly in person** - at the Registrar’s Office, Room 201, Township of Evesham Municipal, 984 Tuckerton Road, Marlton, NJ 08053 between 8:45 am to 4:15 pm. Monday through Thursday.

(**NOTE: Registrar’s Office is closed on Fridays.)

The fee per each request form submitted is $25 for the initial issued certificate (and $5 a piece for each additionally issued same certificate).  Payment is by Cash or Check only. 

• If paying by cash in person, only denominations of $20 bills & under are accepted.

• If paying by check, please make check payable to “Township of Evesham”
